Model and Strategy
The Hospital Engagement + Linkages Project (HELP) is a collaboration led by HealthRIGHT 360, partnering with UCSF Medical Center, that bridges people with substance use disorder to effective treatment. The project places HealthRIGHT 360 Behavioral Health Addiction Specialists at the hospital setting to engage with individuals who can best be served at a community treatment facility. The Specialists conduct immediate assessment and work one-on-one in a compassionate and supportive manner to encourage and facilitate pursuit of treatment. In collaboration with the individual, a comprehensive treatment plan is completed. For people who are ready to enter withdrawal management or residential treatment and consent voluntarily, the Addiction Specialists team will use real time information on available detoxification and treatment options and provide safe transportation to the appropriate resource.
HELP engages people with problem substance use, empowers them to act, and helps them access effective residential treatment to recover and heal from their disease and become contributing members of the community.
Impact
HELP will get people with problem substance use into effective residential treatment and off city streets.The project fills a critical gap in the current system of care by providing access to treatment services from 5 p.m. to midnight.
Goals and outcomes during two-year grant period:
- 400 people will be referred from UCSF medical personnel and evaluated by project staff. 150 will be voluntarily admitted to residential evidence-based treatment.
- Of those initiating treatment, 100% will receive a medical and dental evaluation at the HealthRIGHT 360’s Integrated Care Center. Individuals determined to have Opioid Use Disorder will be evaluated for Medication Assisted Treatment (MAT) and prescribed Suboxone or other medication as appropriate. Assessment for mental health needs (i.e. depression and trauma) will also be conducted.
- 100% will complete a client-driven treatment plan that incorporates assessment results and integrates physical and psycho-social and emotional care issues. Substance use disorder treatment will include evidence-based individual and group modalities. All those in residential treatment will have access to the Integrated Care Center’s primary medical and dental care and comprehensive Resource Center to address housing, employment, education and other personal needs.
- 75% of those entering treatment will successfully complete their treatment goals and transition into stable housing, such as HealthRIGHT 360’s longer term step-down program, supportive housing, sober living residence, or other settings.
